I don't have actual pics or brand name but it is VERY similar to this:

I bought it new from Sports Authority (or whatever they were called back then) a few years ago.
 
Has several stations all built into one unit:
- bench press
- leg extension
- lat pull down
- butterfly
 
No major wear and tear on the unit- looks brand new minus the dust on it.
 
If needed, will deliver and set up in the Wichita area.
